Our doctors will review the medical report application and determine the most appropriate type of medical report to be provided.
Common types of medical reports:

The report may require a doctor’s opinion and/or doctor’s assessment of the patient. When assessment of the patient is required, appointment(s) with the patient will be arranged. Forensic Report The report requires a doctor’s opinion that is related to forensic matters and requires the doctor’s assessments/interviews with the patient and/or his/her family. Medical Report under the Mental Capacity Act (MCA) Details of the Mental Capacity Act can be obtained from the Office of Public Guardian website. The report requires our doctor to complete Form 61 under the Mental Capacity Act. The application for this report often comes with the processing of the Affidavit upon the completion of the Form 61 report. Expert Opinion / Second Opinion Report The report requires our doctor's expert opinion in relation to a professional subject matter or requires our doctor to comment on other reports by private psychiatrists. | $5450 | minimum 12 weeks |
**Depending on the type of report and complexity of the case, the following are situations that would affect the turnaround time on the completion of a medical report.
- The patient is receiving inpatient treatment when the report application is being made.
- The doctor is on leave or away for conference.
- Availability of doctor and patient for assessment.
- Further assessment(s) of the patient before the doctor can complete the report.
The doctor who is preparing the report may require the patient to go through some assessments/tests. The assessment fees are separate from the payment of the medical report fee.

Full payment must be made before the medical report can be released to the patient/ requester. If there is a shortfall of the medical report fee, a top-up payment is required. If the payment made exceeds the medical report fee, a refund will be processed by IMH Finance Department and the turnaround time for refund is about 2 months.

For patient who has difficulty with the Forensic Report Fee, we suggest for patient to check with the Singapore Public Defender’s Office for case assessment and support.
For patient who has difficulty paying the Ordinary Report and Specialist Reports fees, a referral will be made to our Medical Social Worker who will assess the patient’s eligibility for financial assistance. Patient will be asked to submit the relevant supporting documents to the Medical Social Worker.
Medical report will be sent via email to the patient / organisation stated in the IMH Patient Consent Form.
For self-collection, only the patient is allowed to self-collect the medical report. Verification through NRIC is required. For collection of medical report authorised by the patient, an authorisation letter from patient is needed.
The medical report is valid for 6 months from the report date. A fresh application is required after the medical report expires.
Please contact the Medical Records Office to check on the status of the medical report. Once the medical report has been processed and doctor has started preparing for the medical report, cancellation is not allowed. Full medical report fee will be applied.
- Cancellation request must be made in writing through a letter or an email, indicating the reason for the cancellation.
- If doctor has not started on the medical report, we will process the cancellation with an administrative fee of $24. The refund will be processed by Finance Department.
Note: The hospital reserves the right to offset the payment made the patient for medical report against the outstanding hospital bills of the patient.

1. When will the timeline for the medical report application process start?
The application will be processed only when full documents which are the official letter, patient’s Consent form and the full payment for the medical report fee are received. The application will then be sent to our Chief Doctor for his/her review and assignment of a Doctor to write the medical report.
2. Why is the validity of a medical report only 6 months?
The patient’s medical condition may change or new symptoms may develop. Doctor may need to reassess the patient to ensure the present condition of the patient is still relevant.
3. Why is a patient’s consent required when I apply for the medical report as the patient’s immediate family member?
The patient’s medical information is confidential. The patient has to give his/her consent to release his/her medical information. For a patient who is below 21 years old, the consent of the patient’s parent or legal guardian is required. You may download the IMH Patient Consent from IMH Website www.imh.com.sg or obtain a copy from IMH Medical Records Office.
4. Why do some medical reports take longer to complete?
Depending on the types of medical reports, some medical reports will require our Doctor to assess the patient for the current mental condition before a medical report can be written. The assessment can be done in one or more sessions. If the patient is receiving inpatient treatment when the medical report application is being made, the doctor will have to wait for the patient’s condition to be stable first before the assessment can commence.
The outpatient appointments for assessments are subject to the patient’s and the doctor’s availability.

5. If I need a medical report urgently, can you expedite it?
Our doctors’ primary focus is to provide psychiatric treatment and care to our patients. Time is needed for our Doctors to prepare the medical reports. We strongly encourage you to apply for a medical report early to meet your timeline. You may refer to the estimated turnaround time of the medical report completion.
