At the Institute of Mental Health, we have case managers who work closely with our patients to follow up on their care, along with a multidisciplinary team of psychiatrists, psychologists, nurses, medical social workers, occupational therapists and community service partners who provide psychosocial services.
Case management is a collaborative process that uses a comprehensive assessment to identify needs and preferences of our patients.
A case manager supports patients in the following ways:
- Help patients meet their goals and needs according to their care plan
- Connect patients to a network of services and resources in the community
- Assist patients if they have questions regarding medications, symptoms, side effects and appointments
- Advise and support patients on handling relationship issues, managing stress with work, school or family, and managing suicidal feelings
